Cancellation Policy

Dependent upon your quoted total - A $25, $50, or $100 booking fee will be required to secure your appointment. The fee amount will be applied towards your service total. If the invoice is not paid within 24 hours of receiving, you will forfeit the requested date and time.

If you need to cancel or reschedule your appointment, please give 24 hours notice. Any appointments canceled or rescheduled after 24 hours will forfeit their fee, and a new booking fee must be made to rebook in the future. No shows will be charged 50% of the service amount. Finely Inked holds the right to not reschedule any no-show appointments. I allow one "reschedule" per appointment. If you need to reschedule more than once, there will be an additional rescheduling fee. 

 

Late Policy 

I get it - things happen! If you are running late to your scheduled appointment, please contact me as soon as possible and I will do my best to accommodate you! However, if you are more than 15 minutes late to your service, I reserve the right to treat it as a same day cancellation and therefore the cancellation policy will apply to your service.

Previous Permanent Makeup

If you have previous work from another artist, you must schedule a consultation appointment prior to scheduling an initial appointment, as well as send clear, well-lit photos for approval. Showing up to your appointment without disclosing previous work and without approval may result in the inability to complete the service and a loss of your booking fee.

PLEASE READ THIS AGREEMENT CAREFULLY AND ASK ANY QUESTIONS IF YOU DO NOT UNDERSTAND ANY PART OF THIS AGREEMENT OR DO NOT UNDERSTAND ANY PORTION ABOUT THE SERVICES THAT FINELY INKED WILL PROVIDE TO YOU.

Parties
This Agreement is made between Finely Inked and you (“you” or “your”).
Services
You will be provided any permanent makeup or body tattoo service that you consent to that is within the scope of Finely Inked’s offered services (“the Service(s)” or “your Services”)). These services include, but are not limited to: permanent makeup on the eyebrows; permanent makeup in the form of eyeliner; and permanent custom body tattoos on a specified area agreed upon between you and Finely Inked .

Inherent Risks
You understand that participating in the Service(s) include inherent risks. These risks are based on the Service(s) selected, as well as your own body. Every person can react differently to the application of a foreign substance to her/his/their body. There are various skin textures and undertones that absorb and retain pigment differently. You agree to advise the provider of your Service(s), before any service begins, of any allergies that you have whether the allergies be topical (skin) or otherwise. You agree to tell the provider of your Service(s), at any time, during the services if you are in pain or feel uncomfortable. You acknowledge that it is possible to have an allergic reaction to pigment used in your Services immediately or at anytime after the Services are provided.
Guarantees
You acknowledge that no promises, guarantees, or warranties have been made to you by Finely Inked
regarding the outcome of your chosen Service(s). You agree to give Finely Inked verbal confirmation of the
pigment color and position/placement of any permanent makeup or body tattoo prior to its application.

Aftercare
You acknowledge that you have been provided with the aftercare instructions for any service you have selected for which such instructions may exist. You understand that if you fail to adhere to the aftercare instructions that you may suffer harm, including but not limited to, a deterioration of the desired effects of the service, such as loss of pigment, infection, or injury. You agree to contact Finely Inked if you have questions about the aftercare instructions provided to you.

***Photo Release***
You authorize Finely Inked to take photos of you for marketing or training purposes, including but not limited to demonstrate the before and after effect of your Service(s). You consent to Finely Inked using these photos in published media, whether digital or print. You acknowledge that Finely Inked is the sole owner of any photos that may be taken. If you do not wish for your photos to be used for advertising/marketing, please inform Finely Inked and it will be noted on your form.

Voluntary Assumption of Risk
You have read this Agreement and understand the risks of participating in the Services with Finely Inked. Your initials and signature below, and your participation in the Services of your choice at Finely Inked illustrates your voluntary participation and assumption of the risks of doing so.
Consent
By signing this Agreement, you agree that Finely Inked may perform the Services that you request to be
performed within the scope of services that Finely Inked provides. YOU AGREE THAT FINELY INKED, EITHER IN WRITING OR THROUGH THE PROVIDER OF YOUR SERVICES, HAS INFORMED YOU OF THE RISKS INHERENT IN ANY SERVICE THAT YOU MAY SELECT. YOU ACKNOWLEDGE THAT THESE RISKS MAY INCLUDE SKIN INJURY . You acknowledge that the pigments used in the Services, although carefully selected and bought through reputable manufacturers, are not FDA approved for any type of tattooing.

Release and Waiver
IN CONSIDERATION FOR THE SERVICE(S) PROVIDED BY FINELY INKED, YOU HEREBY
RELEASE FINELY INKED, ITS OWNER, AGENTS, EXECUTORS,
ADMINISTRATORS, SUCCESSORS, FAMILY MEMBERS, AND ASSIGNS (“THE RELEASED PARTIES”)
FROM ANY LIABILITY AND DAMAGES ARISING FROM ANY INJURY OR DEATH CAUSED BY OR
ASSOCIATED WITH THE SERVICES PROVIDED BY FINELY INKED. You are releasing the released
parties at your own risk, and you agree to forfeit any and all forms of legal recourse which may be available to you, including but not limited to any form of damages, as a result of your participation in the Services. You agree that these provisions above apply to you, your family, heirs, executors or anyone else who may be able to bring a legal action on your behalf in the future. You acknowledge that you are signing this Agreement of your own free will. You acknowledge that you are at least eighteen years of age and competent to make your own decisions, or you are the parent/ guardian on the minor child receiving the Services.

General Legal Provisions Jurisdiction. This Waiver will be governed exclusively by the laws of the State of Ohio.

Severability. If any provisions of this Waiver are invalid or unenforceable, the other provisions in this Waiver will remain in full force and effect.

Entire Agreement. This Waiver constitutes the entire agreement between the parties and replaces any prior agreements, promises, or commitments.
Headings. The headings used in this Waiver are for stylistic purposes only and none of the content in the headings are intended to be legally binding.
